Endoscopic Transcanal Cartilage Myringoplasty with and Without Tympano Meatal Flap Elevation

Abstract

Background: Perforation of the chronic Tympanic Membrane (TM) is a common consequence of trauma and Chronic Otitis Media(COM) in adults. Tympanoplasty is a surgical procedure that repairs perforations in the tympanic membrane and restores hearing. Aim and objectives: To compare the double surgical procedures of endoscopic transcanal Myringoplasty using tragal cartilage graft with or without elevation of the tympanomeatal flap. Patients and Methods: This study was a comparative prospective clinical study in which forty suitable cases with chronic otitis media with perforation (safe type) and non-healed traumatic perforation were split into two groups after visiting our clinic for six months. From March 2022 to March 2023, it was conducted at AL-Zahraa University Hospital's Otorhinolaryngology Department and the Hearing and Speech Institute. Results: There was no statistically significant difference between the two groups in terms of age, gender, side and amount of perforation, pre-and post-surgery ABG, and success rate. There were statistically significant differences in air-bone gap (ABG) between pre- and post-surgery in each group. The group without elevation had a much reduced operative time than the group with elevation. Conclusion: Trans tympanic tympanoplasty is a straightforward and rapid approach with a high short- and long-term graft success rate, satisfactory hearing outcomes, and low invasiveness compared to the tympanomeatal flap elevation technique. Transcanal endoscopic trans tympanic cartilage myringoplasty is a trusted approach for Myringoplasty and is deemed considerably more efficient in accordance with the ideals of minimally invasive surgical treatments.
