Background: Ankle impingement syndromes are frequent and substantial post-traumatic reasons for morbidity in active people and athletes. Aim: To evaluate the Ultrasonography (US) role compared to Magnetic Resonance Imaging (MRI) as a benchmark in impingement syndromes of the ankle diagnosis. Patients and methods: This research involved 30 cases; they were referred from Sayed Galal Hospital Outpatient Clinics to the Radiology Department, Sayed Galal Hospital, Al Azhar University, during the period August 2021 to February 2024. Results: Study results show that the incidence and percentage of total impingement lesions diagnosed through Ultrasonography was 39 (76.5 %) & 51 (100%) by MRI with P-value 0.001, the percentage & frequency of bony impingement lesions diagnosed through Ultrasonography were 27 (100%) & 19 (70.4 %) by MRI, the percentage and frequency of soft tissue impingement lesions diagnosed through Ultrasonography have been 24 (100%)& 20 (83.3 %) through MRI, Of the 27 bony impingement lesions that were diagnosed through Magnetic Resonance Imaging, an ultrasound indicated 19 (n=19/27, 70.4 percent) bony lesions with specificity 100 %& sensitivity 70.37 % respectively (p-value = 0.001). Conclusion: Ultrasonography is a valuable diagnostic instrument for soft tissue and bone impingements, saving time and cost and detecting further irregularities which might mimic impingement symptoms.
