Assessment of the Diagnostic Accuracy of different Pulse Sequences of Multi-Parametric Prostate MRI in the Diagnosis of Prostate Cancer in Comparison to Trans-Rectal Ultrasound Guided Biopsy

Abstract

Background: Many men have been underdiagnosed or missed diagnoses due to the present prostate cancer diagnostic method, while others have been overdiagnosed and treated excessively as a result. One technique that could help reduce these kinds of mistakes in diagnosis is multi-parametric magnetic resonance imaging (mp-MRI) of the prostate. Aim and objectives: To evaluate the histopathological findings from TRUS-guided prostate biopsies with the diagnostic accuracy of various mp-MRI pulse sequences for the accurate detection, localization, and staging of prostate cancer. Patients and methods: Fifty patients were included in the study; they were sent from the Urosurgery clinic and department at Al-Hussein hospital to the MRI unit of the Radiodiagnosis department at Al-Azhar University in Cairo. From September 2022 through September 2024, the research was carried out. Results: With the PI-RADS v2 scoring system, mp-MRI had an accuracy rate of 88.9%, a specificity rate of 84.6%, and a sensitivity rate of 91.3% when diagnosing localized lesions in the prostate. Conclusion: The diagnostic use of prostate Mp-MRI with the PI-RADS v2 scoring system is quickly growing, and it shows great promise. It is a reliable and non-invasive method for characterizing localized lesions in the prostate and differentiating between benign and malignant ones. PI-RAD V2 score, in conjunction with Mp-MRI, T2WI, and ADC measures, is a precise and sensitive imaging method for characterizing localized lesions in the prostate and differentiating between benign and malignant lesions.
